If your baby appears ill, listless, feverish,  lethargic, fussy and/or is not wetting at least 6 times in 24 hours, contact  your baby's doctor or local emergency room immediately.

If the recommendations made by Anne Smith, BA, IBCLC via Internet/Email do not correct the problem or the problem gets worse, you MUST call your own doctors, medical providers, or lactation consultant for a one on one consultation and further advice.

General questions about nursing and pumping can be answered  without going into a detailed medical history. However, if you have a specific  problem, filling out a medical history form will give me the information I need to help you with your problem, and also to decide whether you need to make an  appointment to see your local health care provider.

This form is lengthy, but if you take time to fill it out, it will be helpful to both of us.  You will be asked to answer the some sort of questions that you would be asked to answer in an office visit with a lactation consultant.  I have made arrangements to guard and store the information off my  hard drive in order to protect your privacy. The information you provide will NEVER be used for any purpose other than providing advice directly to you  regarding your particular breastfeeding problem, and will NEVER be shared with anyone without your written permission (for example, if you wanted to have the  information sent to your health care provider).
